Bring Your Blood Sugar Back Into Range
If your A1C continues rising, your numbers swing throughout the day, or your current plan feels inconsistent, it’s time for a closer look.
At Quest in Bay Shore, we treat Type 1 diabetes, Type 2 diabetes, and prediabetes. Rather than looking at a single reading, we evaluate your full pattern: medication timing, daily fluctuations, and how your body responds over time. Then we design a plan built to steady your glucose and move your numbers toward target.
What We Help With
- A1C above target
- Frequent highs or sudden lows Medication-resistant high blood sugar
- Morning blood sugar spikes Insulin adjustments Continuous glucose monitor support
- Early nerve or circulation concerns
What Your Visit Includes
We begin with a detailed review of your recent labs and glucose trends. From there, we refine your treatment strategy and set clear targets for the months ahead.
You leave with
- Defined numeric targets
- A structured adjustment plan
- A scheduled follow-up checkpoint
The focus is steadier daily energy, fewer swings, and measurable improvement in A1C over time.
Stabilize Your Blood Pressure and Feel Steady Again
If you feel lightheaded when standing, notice sudden drops in your readings, or your blood pressure remains unpredictable, a detailed evaluation can bring clarity.
At Quest in Bay Shore, we look beyond isolated readings. We evaluate hypertension, medication-resistant patterns, positional changes, and how your cardiovascular system responds to movement and stress. Then we build a stabilization plan designed to restore consistency.
What We Help With
- Sudden drops in blood pressure
- Dizziness or lightheadedness when standing
- Post-meal blood pressure changes
- Medication-related fluctuations
- Chronic low blood pressure
- Fatigue linked to circulation changes
- High blood pressure despite multiple medications
- Hypertension management and adjustment
What Your Visit Looks Like
We begin with a careful review of your recent readings, symptoms, and medication history. From there, we assess how your blood pressure responds in different positions and refine your treatment strategy with clear stabilization goals.
You leave with
- Clear explanation of the fluctuation
- Specific stabilization recommendations
- Defined monitoring and follow-up plan
The focus is consistent regulation, fewer disruptive episodes, improved clarity, and steady blood pressure over time.

My daughter just left there in tears after being seen by Mithra Toughanian. My daughter has not been feeling well and went there to be seen. She literally told this woman that she has anxiety about Dr's and to please understand this during her appt. All this FNP did was be insulting towards my daughter. Didn't even bother to listen to what was bothering her and just kept telling her she needs to see someone about her anxiety?? Extremely rude. How do you have some one there that makes patients feel worse than they already do?? Its pretty appalling, to say the least. Don't bother going here. Better yet, if you get an appointment with that creature, cancel it. I'm sure there are much better facilities around that wont try and gas light youPosted on fanny baleonTrustindex verifies that the original source of the review is Google. Thanks to a friend who recommended me to come here, I am grateful to the doctors. I am much better now. They helped me so much to control my illness. It is the way they treat you, the attention they give you is excellent. Thank you very much.Posted on R BTrustindex verifies that the original source of the review is Google. I’ve been coming to this doctors office since I grew out of my Pediatrics. I can’t honestly seem to put in words how much I appreciate this office and their staff since not only do I trust them with my health, but also my families. As I do have other family members in my house that are patients there as well . Starting off with how well communication works with the lovely women upfront Natasha ! To the PA Garcia !!! PA Garcia shows so much empathy and compassion for his patients like myself who’s very self centered and introvert . He’s shown nothing but kindness and understanding during my visits . He’s helped me overcome alot of health problems I’ve faced and still does till this day ! I can’t thank him enough for all the support and ofc Natasha as well !! ❣️Posted on Darianny PeraltaTrustindex verifies that the original source of the review is Google. It's a shame how this office has deteriorated with the years. I've been a patient of Dr. Garcia for a few years now. He is the best PA I ever seen. But the office itself it's just bad. No AC during summer and no heating during winter. I feel so bad for the employees that works there. I love everyone in the office but if this continues happening I would have to have PCP for my whole family that goes there and also I will ask my friends to STOP going to the office.Posted on Honoralia FernandezTrustindex verifies that the original source of the review is Google. In all my years going to this office, I can say with certainty how bad has gotten. Providers and staff are nice and helpful. But the office, the office itself it’s a whole different story. I’ve been going there for over 20 years, when Dr Rodriguez was the doctor there; and let me tell you, what a difference. He would’ve never had allowed for this office to get to this point. No AC!!! With how extreme the temperatures have been. My daughter also goes there, she had an appointment last Friday, she came home complaining of how hot was in there. She felt she was about to pass out and felt sick to her stomach.Posted on Oscar LazoTrustindex verifies that the original source of the review is Google. The staff and providers are always nice. Very helpful. The only reason, I give them one stars is because lately the office is extremely hot. It's too uncomfortable to be there.Posted on antony romeroTrustindex verifies that the original source of the review is Google. It's a shame how this office has deteriorated with the years. I've been a patient of Dr. Garcia for a few years now. He is great. But the office itself it's just bad. He is very attentive to my needs and concerns. My only issue as of late is how hot it’s been in that office. I was there just yesterday and I felt I was about to pass out. I don’t understand what’s happening.Posted on Brandon ConeTrustindex verifies that the original source of the review is Google. When it comes to Dr. Gian Carlo Garcia you will not finder a finer doctor nor man to help you. From the first time I had an appointment with him I immediately could sense how professional he was. At the time I was using him as a backup doctor however my doctor left, because unfortunately they do have a high turnover rate for doctors there. Which sucks but change is inevitable. Anyhow he has since become my primary doctor and I've learned he's served his country, so not only does he help people like myself on an almost daily basis he also signed up to fight for our country. If you need a doctor come here and see him. Dr DiCanio is also a good doctor in his own right. He handles things a little differently but I think that's because he's in charge and has to keep oversight strict. Anyway I love Quest Medical. And to give you some idea I live in Harlem. I literally commute an hour and 20 minutes each way once a month just to go there practice. And I won't change. I love the staff upfront from my main gal Rosa to the lab tech and Office Supervisor. I may not know everyone's names but everyone's friendly and they treat me right. I hope this place stays open forever so people like myself will have a place they can always use. Well wishes to everyone

Common Questions About Diabetes, Hypertension & Blood Pressure Care
Patients in Bay Shore and throughout Suffolk County often ask the following when evaluating blood sugar or blood pressure concerns.
At Quest in Bay Shore, we look beyond isolated readings. We evaluate hypertension, medication-resistant patterns, positional changes, and how your cardiovascular system responds to movement and stress. Then we build a stabilization plan designed to restore consistency.
How is diabetes diagnosed?
Diabetes is diagnosed with blood tests such as the A1C, fasting glucose, or oral glucose tolerance test. An 0A1C of 6.5% or higher on two separate tests confirms diabetes.
What A1C level is considered diabetic?
An A1C of 6.5% or higher on two separate tests is considered diabetic. Levels between 5.7% and 6.4% indicate prediabetes. For many adults with diabetes, treatment goals aim to maintain an A1C below 7%, though targets vary based on age, health history, and risk factors. Persistent elevation warrants structured medical evaluation.
What are the symptoms of diabetes?
Common symptoms of diabetes include increased thirst, frequent urination, fatigue, blurred vision, and slow-healing wounds. Some individuals experience minimal or gradual symptoms, which makes routine screening important. Ongoing changes in energy levels, vision, or glucose readings should be evaluated to determine whether diabetes or prediabetes is present.
What is considered low blood pressure (hypotension)?
Hypotension is defined as blood pressure below 90/60 mmHg. The clinical significance depends on symptoms and overall health status. Some individuals tolerate lower readings without difficulty, while others experience dizziness or weakness. Evaluation focuses on both the number and how the body responds during daily activity and position changes.
What is considered high blood pressure (hypertension)?
High blood pressure, or hypertension, is generally defined as a reading of 130/80 mmHg or higher. Persistent elevation increases the risk of heart disease, stroke, and kidney complications. Diagnosis is based on repeated measurements over time rather than a single reading. Structured management helps reduce long-term cardiovascular risk.
What are the symptoms of hypotension?
Symptoms of hypotension may include lightheadedness, dizziness when standing, blurred vision, fatigue, nausea, or fainting. These episodes can occur with dehydration, medication changes, or autonomic imbalance. Recurrent or severe symptoms should be medically assessed to identify the cause and stabilize blood pressure patterns.
When should I see a specialist for blood sugar or blood pressure concerns?
You should see a specialist if your A1C remains above target, your blood sugar fluctuates despite treatment, or you experience repeated dizziness or sudden blood pressure drops. A focused evaluation provides deeper analysis, medication review, and structured follow-up. Quest offers diabetes and blood pressure care in Bay Shore designed for measurable control and long-term stability.
